Insurance Plans

We accept most insurance plans and will gladly process your claim. As a complimentary service we contact your insurance company for details about your plan. This aids us in maximizing your insurance benefits.

Please bring the following information with you on your first visit.

• Insurance card if one was provided to you
• The subscriber's ID number. If you do not have this number, please provide the subscriber's social security number.
• Insurance policies generally cover only a portion of the total treatment cost. Once we have obtained details of your insurance coverage, we ask that you pay your estimated portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.

For your convenience, we also accept payments made with your Visa, MasterCard, Discover, and American Express. Financing is available through Care Credit.
